My question has been put on hold citing a rule that I think I haven't broken. Is there any way to appeal the decision?
Do note that questions that are edited shortly after being put on hold are placed in a review queue to consider them for potential reopening. In this case, it was reviewed, but the reviewers decided not to reopen it.

Moderators can clear migration histories, so if you spot one that you really think is unnecessary, flag for a moderator to clear the migration history.

Bug permits me to flag an answer as "not an answer" multiple times
@TwistyImpersonator Typing "not an answer" into the custom flag field on a post where one has already cast a standard NAA flag will give an error message instead of allowing one to submit a second NAA flag.

Note that since this question was asked, the system now permits banned users to ask one question per 6 months, beginning 6 months after the ban was imposed.

@Vi. The untagged tag is now blacklisted across the network, which means it's impossible for regular users to use it: it can only be applied per the special circumstances above (which do happen, by the way)

As of a few years ago, it's no longer the case that mods have to dismiss all pending flags with the same outcome. So it must have been the case that the mod handling the flag pressed the wrong button.

By this token, Hackintosh questions would be considered on-topic, whereas that specific topic has been debated here before and the consensus was to declare it off-topic.
